We provide better access to care via 24/7 support system including access to health coaches, telemedical visits, and secure messaging to coordinate your care, understand medication compliance, and prepare for medical appointments.
We call each patient discharged from the hospital to make sure the doctors instructions are clear, address follow up care issues, and assist with questions.
If you are an established patient of one of our providers; our physicians may, in some cases, permit refills of your prescription by phone. To do this, call your pharmacy, who will fax your request to us with the appropriate information. Please allow 72 hours for refills.
If you are on maintenance medication; high blood pressure, diabetes, seizure medication and etc., we require frequent visits by you to ensure your health and safety.
